Each applicant will need a digital photograph for the application purpose. This photograph must be as per the specific given criteria.
Make sure that the photograph follows these guidelines.
Here is a highlights of the requirements.
1. Picture Dimension 2 by 2 inch.
2. Must be in white background.
3. High quality digital picture minimum of 300 DPI resolution.
4. Must be saved in .JPG format.
A good photographer can get this picture if you mention Photograph needed for US visa purpose.
Note: Failing to have a proper picture in the right format, can delay application process or have adverse effect on yo your visa.
2. Complete US Visa application form DS160
US visa application form is known as Form DS160.
This application must be completed online , over the internet. There is no paper application available.
You can complete this application here at Dept of State web site.
After the successful completion you will receive a confirmation page with barCode. Take a neat print of this page, this will be needed for setting up visa interview appointment at your applicable US consulate or embassy.
It is OK for any one can complete the application for you. You must check and verify the information for its correctness.
Note: Your application confirmation page should be printed using a laser printer, as ink-jet or dot-matrix printers do not print with sufficient resolution for the bar code to work.
3. Pay Visa fee
You must pay US Visa. Current visa fee is $140 or its equipment in your home country currency. Check with your nearest US embassy/consulate for details.
Where and how to pay ?
While some consulate wants you to make the payment for fee prior to the visa interview appointment, some may want you to make the fee payment at the time of visa interview.

5. Appear for the US Visa interview
Each visa applicant except minors are required to appear in person for Visa interview. You will be required to appear for a visa interview at applicable US consulate where you have applied for your US visa application.
